Sapphire Firefly spawns as a level 25 in the Underbelly as a joining pet when fighting the Blind Rat or the Mutant Warturtles in Legion.

A Sapphire Firefly (poor quality but for the rarity factor I abandoned the blue blind rat...) spawned as an additional pet in one of my Blind Rat battles.

 

Will continue to research this mechanic 

 

Update: firefly seems to spawn frequently as an additional with all Underbelly battles including Mutant War Turtles So don't spend hours in SoA
